Three people died Sept. 6 after a Honda Pilot rear-ended a stopped truck hauling oxygen tanks on the I-66 Express Lanes in Fairfax County, Virginia State Police said.

The crash happened at about 4:10 p.m. at mile marker 64.3 in the westbound express lanes, near the I-495 interchange and the Dunn Loring-Merrifield Metro station. The Honda caught fire on impact. All three occupants were pronounced dead at the scene.

VSP initially reported one fatality. On Sept. 7, officials confirmed two additional people had been inside the Honda, bringing the death toll to three.

The identities of the three people killed had not been released as of Monday. VSP said troopers were working to notify next of kin.

Investigators determined a Toyota 4Runner was also involved in the events leading up to the collision, according to WUSA9.

After consulting with the Commonwealth's Attorney's Office, state police charged both the Toyota driver and the truck driver with reckless driving. Neither driver's name has been released.

Aspen Fetty, an eyewitness who spoke to WJLA, described seeing the Honda erupt in flames. "That was by far the most devastating thing I've ever witnessed," Fetty said.

The westbound express lanes were closed for nearly seven hours and reopened just before 11 p.m. Sunday. The stretch of I-66 near mile marker 64 is a heavily used commuter corridor for Vienna-area residents traveling to and from Washington.

Virginia State Police said on X on Sept. 6 that the scene was active and troopers were working to identify the deceased. The crash remains under investigation by the VSP Fairfax Division. Anyone with information can contact Virginia State Police.
